Millstone Medical Outsourcing, Inc.
("Millstone") is strongly committed to respecting

person accessing, browsing or otherwise using this website located at www.millstonemedical.com (the
“Site”), either manually or via an automated device or program, shall be
considered a “User.” The purpose of this
Privacy Policy (“Policy) is to explain the type of information that Millstone collects from our Users, what
Millstone does with that information, and with whom Millstone shares this

User Consent to Privacy Policy

Site, Users, including Extranet Users (defined below), acknowledge that they
have read, understand, agree and consent to the terms and conditions of the
Policy. Users consent to the collection,
use, and disclosure of User information including personal information and
anonymous browsing information (together “Information”) pursuant to the terms
of this Policy. If you do not consent to these terms and conditions, please do not
access, browse, or use this Site or provide Information to Millstone.

MILLSTONE COLLECTS
Anonymous
Browsing Information
Users
can browse the Site without revealing personal information. In this
context, our servers may collect your Internet Protocol address, your
computer’s operating system, the name of the domain you used to access the
Internet, the website you came from and the website you visit next. See "Cookies" section below for
more information about anonymous browsing information.
Personal Information
Millstone may collect the name, address, company
name, phone number, e-mail address, credit card information and other personal
information provided by a user of the Millstone Online Field Inventory Ordering
System or Extranet (an “Extranet User”). Millstone may also collect any
Information provided by a User to Millstone while using the Site.
By submitting Information to this
Site, Users and Extranet Users acknowledge that they have read this Policy,
understand it, agree to its terms and authorize Millstone to collect, use and
disclose Information pursuant to the terms of this Policy.
COOKIES
A "Cookie" is a piece of
information that is sent from the Millstone web server to a User's browser and
stored on the User's hard drive.
Currently, Millstone uses Cookies when a Extranet User orders a product.
The Cookie stores information that allows the Site to remember the Extranet
User the next time he or she visits the Site. Millstone's cookies do not collect any Personal
Information and do not store any credit card information. You may set your Internet
web browser to notify you when you receive a cookie or to prevent cookies from
being sent. If you prevent a cookie from being sent, you may limit the
functionality Millstone can provide you when you visit the Site, especially
when using the Extranet.
How MILLSTONE Uses the Information
Millstone
uses the Information collected from its Users to respond to Users’ questions
and/or comments, to process an Extranet User’s request for a product or
information, or to provide Extranet Users with important service-related information.
Anonymous
browsing information may be used to gather broad demographic information used
in marketing, promotion or similar activities.
This information may be aggregated to measure the number of visits,
average time spent, page views and other statistics about Users of the
Site. Millstone also may use this data to monitor Site performance and to
make the Site easier and more convenient to use.
Who MILLSTONE shares your Information
With
Millstone
will not sell, share, trade or give away any Information that it collects or
receives regarding its Users subject to the following exceptions:
· If
Millstone has a User's consent to share personal information, it may do so.
· Millstone
may provide to others the aggregate statistics about our Users' site activity
for purposes of marketing, promotion or similar activities. None of these
statistics will identify Users personally.
· Millstone
may disclose Information about our Users to others if we have a good faith
belief that we are required or permitted to do so by law or legal process, to
respond to claims, or to protect the rights, property or safety of Millstone or
others.
· In
the event that Millstone decides to sell all or part or its assets, Millstone
reserves the right to include Information among the assets transferred to the
acquiring company.
UPDATING INFORMATION
Millstone offers Extranet Users the
opportunity to review the Information provided to Millstone to update it or
correct any errors. To update or delete Information previously provided to
Millstone, call 508-679-8384 ext. 2 or send an email to sales@millstonemedical.com with
"Updating Extranet Information" in the subject line.
Applicable Law/International Issues
Information that is submitted to
this Site will be collected, processed, stored, disclosed and disposed of in
accordance with applicable U.S. law. If you are a non-U.S. User, you
acknowledge and agree that we may collect and use your Information and disclose
it to other entities outside your resident jurisdiction, as discussed in “Who
Millstone Shares Your Information With” above. In addition, such
Information may be stored on servers located outside your resident
jurisdiction. U.S. law may not provide the degree of protection for personal
information that is available in other countries. By providing us with
your Information, you acknowledge that your have read this Policy, understand
it, agree to its terms and consent to the transfer of such information outside
your resident jurisdiction. If you do not consent to the terms of this

The Site provides hyperlinks, which
are highlighted words or pictures within a hypertext document that, when
clicked, take you to another place within the document, to another document
altogether, or to other websites not controlled by Millstone. These

those provided herein. Millstone is not responsible for the collection,
use, or disclosure of information collected through these websites, and
Millstone expressly disclaims any and all liabilities related to such
collection, use, or disclosure.
Children’s
Privacy Protection
The Site is not directed towards
children under 13 years of age, and Millstone does not knowingly collect any
information from children under 13 years of age through the Site. If you are under 13 years of age, you are not permitted to
submit Information to Millstone through the Site.
Amendments

Millstone reserves the right to
amend this Policy periodically. Any amendments made to this Policy will
be considered effective immediately after the amendments are posted on the
